Summary

Aging adults strive to remain healthy at home and in their community, but sometimes need help to accomplish this goal—this is where preventative population health programs inclusive of HOP-UP-PT (Home-based Older Persons Upstreaming Prevention Physical Therapy) can help. The mission of this program is to provide early preventative interventions to senior citizens at risk of being homebound by facilitating partnerships between community centers and local physical therapists. The goal of this course is to provide education to therapists in order to administer the HOP-UP-PT program so they can join others in preventing senior citizens from becoming homebound. Participants of this course will learn about treatment referrals, aging philosophy, key evaluation tools, and treatment strategies.
